I had same problem. found that the #4 injector was leaking. replaced it and seals . Problem solved this was on a 2004 with just over 58k mileage dropped from 42-44mpg to 36 - 38 mpg after tbs valve adjustment airfilter spark plugs and new injector , mileage back up to 42+
