27/19mm combo tool

I'm sure some of you folks have already solved this; I would like to have a box combo wrench with 27mm on one end and 19mm on the other, small and light to carry on the bike in case I'm out in Left Nut and need to change a tire. What do you IBA types carry with you for that purpose??

I carry a 3/8 socket set with me. Why is that relevant? I also carry a 1/2" drive 27mm socket and a 3/8 to 1/2" adapter. For the front, I'm one of the cheap guys that uses a spark plug wrench with the closed end fitting snugly into the 19mm end of the axle. The 3/8 ratchet and a 2" extension fits through the spark plug socket.
