For a rebate you must submit the completed rebate form, original sales receipt or copy (see above) and UPC barcode, w hich can be
found on the packaging. Rebates must be postmarked within 30 days of purchase date (not delivery date). Limit 1 rebate
per household, per address, per person, per receipt. Valid in the United States and Canada only, and not with any other offer.
Requests from PO Boxes, groups or organizations, will not be honored or acknowledged. This right cannot be transferred, and void
where taxed or restricted by law. Rebates are paid in US funds only and should arrive 8-10 weeks from receipt of rebate form. Connect3D,
www.buy.com and their fulfillment companies are not responsible for lost, mutilated,
misdirected or postage-due requests. Use of fictitious names or addresses could result in federal prosecution for U.S. mail fraud.
This offer is no longer valid if not fully redeemed within 11 months from the close of the redemption period. Make a photocopy of all
materials submitted as Connect3D requires all original documents and will not return said items. Connect3D will stop
processing and reject any rebate request not complying with these instructions. If 10 weeks have passed since submitting a rebate
