Are these just hand tightened or are there flats on shaft to accommodate a wrench?

They are just hand tightened. I put thread lock on mine, and I don't think I could take it off very easily... its nice that I know I could take it off if I wanted to, but it fits tight enough to not worry about it.

Red locktite will keep it securely in place until you heat it up (you'll probably destroy the rubber ball at that point).

A small dab of Blue Locktite would be better than red . . . wouldn't it? Was actually thinking about that today when I put mine on.

That is what I did. Blue locktite. It worked great an feels sturdy. -

It depends on how permanent you want it.

If you just want it to not come undone while you are underway, but would like the option of being able to easily remove it, then blue is the stuff you want.

But if your goal was to make it a permanent fixture so that miscreants can't just unscrew it and steal it (along with all of the valuable stuff you have mounted on it) then red would be the premium choice.

I guess you have to ask yourself: "Once I put this ball on here am I ever going to want to take it off again?"
