Going down hard is really tough. Been there, done that, but not as bad.
I wish you a full recovery and offer this advice: Attack physical therapy like it's an enemy to be defeated and the therapists are on your side. Do EVERYTHING they ask, no matter how much it hurts. Push as hard as you can, as long as you can, until you can't stand it. Don't give up--recovery and progress takes many months. Many days you just DON'T see it happening and that's discouraging. You'll get depressed. That's normal, so expect it.
I'm not an especially competetive, aggressive person, but in P/T I became an animal, and STILL felt like I didn't press hard enough. With all the weight lifting they made me do, and the upper body cycle (arm bike) the therapists at times would MAKE me back off to ensure my form was correct, because I wanted to push so hard.
Today I have 90-95% of my shoulder motion back, can bang out the pushups in the dojo, and can land and take all the punches with that arm, fearlessly.
It was the PT that did it.
