She's almost certainly not sick or immunosuppressed like you would think if you saw someone in the US wearing a medical mask. Many people over there wear these masks in the belief that their allergies will be helped. It is really strange to see this on the streets, trains or subways. Businessmen in expensive suits with a cheap mask, young girls dressed like Disney characters with a cheap mask, etc.
A few years ago I asked a Japanese allergist if this is effective. He replied, "Not really, but it makes them feel like they are doing something."
